I/We, the undersigned, are the parent(s)guardian(s) of the above-named child(ren), and we agree, in taking advantage of the child care services provided by Anywhere Childcare, to release and hold harmless Anywhere Childcare, its officers, directors, agents, employees, volunteers, and families from any and all claims, demands, suits, costs, and charges in connection with or arising out of the child care service, including, but not limited to, bodily harm or injury to our child(ren), except only for loss, harm, or injury occasioned by gross negligence or intentional misconduct by Anywhere Childcare and/or its officers, agents, employees, volunteers, and families.
